(An expert opinion blames the flight crew).
The Rio-Par flight which claimed 228 lives in 01 June 2009 was a flight crew error according to a counter- expert opinion. Up to now the findings have stated technical concerns on flight AF447 and adverse weather. The counter-expert opinion suggests that the accident was due to “an inappropriate action of the flight crew”.
The AF accident “could have been avoided”, according to the conclusions in a report by a counter-expert from AFP made known on Tuesday. The catastrophe, which cost 228 lives, was due to a “an inappropriate reaction of the flight crew after the speed indications showed loss of momentum.
The experts affirmed that the simulations “clearly establish the predominance of human factors in the causes of the accident and in contributing factors”, and following on, “we have also determined that the accident could have been avoided, by some appropriate actions of the crew”.
Rien de nouveau pour Air France (Nothing new for AF)
The lawyers close to the victims said of the report “Full of contradictions and inaccuracies” “The experts seem content on blaming the pilots surrounding the study of the central question of technical weaknesses”. AF deplores “these leaks are amazing on the counter-expert opinions demanded by Airbus.” but estimated that a “first partial reading tends to carry hardly any new elements.”
In this counter-expert opinion, ordered by the justice in 2013, the authors compiled a list of 14 factors in order of importance. “The absence of structured analysis of the fault present”, and overall, “mis-understanding on the situation and “the activation of the tasks in the cockpit which were not applied in a rigorous manner”
The Experts put equally a cause on AF. There is, according to them, an “absence of clear directives on the part of AF, despite many known cases of ice on the pitot probes and insufficient feedback experience”. They estimated that the pilots were not sufficiently informed “in the application of procedures during Indicated Air Speed (possible) inaccuracies” required during icing of the probes and aircraft handling during loss of speed indications.
In July 2012, a BEA Bureau d’enquêtes et d’analyses report already detailed a similar thesis: the accident could have been as a result of a combination of a technical problem with the pitot probes indicating airspeed and pilot error.
